Death Appointment

By law you must register a death within five calendar days unless the coroner is investigating.

The death cannot be registered until the doctor or hospital provides a medical certificate of cause of death or, if the coroner is involved, the relevant paperwork has been provided to us.

The closest relative must register the death (unfortunately statute does not regard an unmarried partner as a relative).

Documents for the relative registering the death, as well as the deceased, must be provided to show names and addresses to prevent any errors.

Please be aware that hospitals in the city of Stoke-on-Trent (including the Royal Stoke University Hospital) and in Sutton Coldfield (including Good Hope Hospital) do not sit within the registration district of Staffordshire.

If the death occurred at home, or elsewhere in Staffordshire, please check that the exact location is within the Staffordshire registration district. Deaths must be registered in the district that they occur, if you are unsure please use the postcode checker to establish a district.

For more information about registering a death please visit our website.

Once your appointment has been made you will be given a reference number; please keep this safe as it will be required if you need to contact us to amend or cancel your appointment.
