Death Registration Appointment Information

A death registration appointment can be made once the Registration Service has recieved the relevant paperwork from the Coroner or Medical Examiner.
 
You will recieve a phone call from a member of our Registration team to book an appointment, or an email / text will be issued, including a link to enable you to book an appointment online.
 
If the death occurred outside of Staffordshire, we are unable to complete the registration locally.  However, we can offer an appointment to attest a declaration.  During the appointment, a Registration Officer will record the necessary information and forward to the Registration District where the death took place, who will complete the registration and issue via post the green form for the funeral directors and copies of the death certificate.

Once your appointment has been made you will be given a reference number; please keep this safe as it will be required if you wish to amend or cancel your appointment.
